You can create custom registration forms to gather additional information about clients and dogs in your system.

Creating Registration Forms

Once you have created your fields, you can create registration forms that will be used to gather additional information from your clients during the online registration process. To create a new form, go to the Admin Module ->Setup ->Online Registration ->Registration Forms page.

Once inside, you would want to click on the 'New Form' button in order to create a new custom form. In the popup, enter the name of your new form and choose whether this is a client or dog form. For a client form, you will be able to choose from all of the client fields you created, same goes for a dog form. Just check the 'Form' box to include a field on this form. If you want the field to be required, meaning your client has to enter a value for this field on this form then check the 'Required' box. 
 

Assigning Forms to the Registration Process

Once you have created a form, you can assign that form to a portion of the registration process. There are several places to do this. If you have a form that you want to be included for every client or dog that is registered in the system you can assign that in the Admin module -> Setup -> Online Registration -> Configuration page. Just choose the Client and/or Dog form and click the 'Save' button at the bottom of the appropriate section.

Classes: If you have specific information that you need for a class, you can create a form and assign that on the Classes Page in the Admin Module. This way each class can have a different form if you so desire. 

Events: For events you can assign a Registration Form by creating at least one Registration Level. When you create a Registration Level you can choose a Client Form and if that level allows a dog, you can also choose a Dog Form. 

Private Training: For Private Training you can choose a Client and Dog form for each separate sign-up option you create. 

Therapy: For Therapy you can choose a Client and Dog form for each separate sign-up option you create. 

Daycare: For Daycare you can choose a Client and Dog form for each separate sign-up option you create.

Boarding: For Boarding you can choose a Client and Dog form for each separate sign-up option you create.

Packages: For Boarding you can choose a Client and Dog form for each separate package you create.
